The formats that you can download the data in are just as comprehensive, and should cover just about anyone’s computer, platform, or preference. They include: Access; Excel; Dbase 5; Delimited, and PDF.

How to find Medicaid Certified Assisted Living in Arizona

You will need to visit the Arizona Department of Health Services, and their Public Health Licensing section. They have a great list of Provider and facility databases.

What I did was go to ‘All Medicaid Certified Providers’ and I went to the PDF document, which is my preference. I was greeted to a very large, 732 page document of Medicaid-Certified facilities, broken down by county.

You can search this document by word, usually by selecting ctrl+f on your keyboard. You might try searching for a city, like Phoenix, or Tucson, to see what’s available, or you may just search the term ‘assisted living’ and then just keep hitting ‘enter’ to scroll through the facilities that accept Medicaid.

It might be easier for some people to open this information in a spreadsheet, such as excel, so you can sort and/or find your information a little easier.

Other ways to search for assisted living and residential care in Arizona

The way I described above is specific to finding Medicaid approved assisted living in Arizona. Another way to search for all licensed care statewide is through the AZ Care Check, which provides links to searchable databases for:

  • Residential Facilities (including Adult Day Health Care, Assisted Living Homes, Adult Foster Care and more)
  • Long Term Care (including Nursing Homes and Immediate Care Facilities, and more).

If you visit the Residential Facilities Provider section you will see a simple search box that you can enter your information and parameters. There are several different types of assisted living homes in Arizona, including:

  • Directed
  • Personal
  • Supervisory
